In preparation for enrollment:
• Please read the 2011 Benefits Enrollment Guide.
• Have the Social Security numbers and birth dates for any spouse or child(ren) that you plan to add to benefit plans.
• Have names, birth dates, address for individuals you plan to list as life insurance beneficiaries.
